Instructions for Faith Formation Registration
To register proceed as follows:

Download one Family Registration Form
for each family.

Download one form for each family member
according to grade
level.
-
Fill
in all information and return to the parish office with
payment.

If you have questions or trouble
downloading the forms contact the person in charge of the appropriate grade
level program.
